Our focus is to provide invaluable education and connections at a community based level. These connections come in many forms:
We will utilize the ASAM co-triage to identify an individual’s needs, work with HARDCORE team members to prioritize those needs, and work with community partners and established service providers to get needs met. There are numerous barriers an individual faces when seeking treatment, HARDCORE connections was made to knock down these barriers.
- Connections to a licensed treatment facility
- Connections to community based supports (housing, food, financial assistance)
- Connections to peer recovery support groups, with many more.
